29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) Jobsite, 200 Main Street, Coraopolis, PA: On or about August 9, 2017 employees performing roofing activities on the flat roof approximately 20 feet above the ground were not protected from falling by guardrail systems, safety nets, personal fall arrest systems or a warning line system with a safety monitor. Employees were removing old asphalt roofing material and tossing it off the roof into dumpsters and no systems were in place to prevent falls.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite, 200 Main Street, Coraopolis, PA: On or about August 9, 2017 the employer did not have a training program to ensure that employees who are potentially exposed to falls were able to recognize the hazards and what procedures or methods could be used to protect themselves or minimize their exposure to such hazards.
