29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Jobsite, 200 Main Street, Coraopolis, PA 15108: On or before September 25, 2017, employees were working unprotected and exposed to fall hazards of approximately 18 feet. The employees were working to replace a damaged roofing system on a low-sloped roof. The Mullenix Construction Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(B)(10), which was contained in OSHA inspection number 1253477, citation number 1, item number 1 and was affirmed as a final order on September 22, 2017, with respect to a workplace located at 200 Main Street, Coraopolis, PA 15108.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite, 200 Main Street, Coraopolis, PA 15108: On or before September 25, 2017, employees were not trained on fall hazards or the proper techniques used to prevent falls from a low-sloped roof with a height of approximately 18 feet. The Mullenix Construction Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1253477, citation number 1, item number 2 and was affirmed as a final order on September 22, 2017, with respect to a workplace located at 200 Main Street, Coraopolis, PA 15108.
