29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, employees were exposed to amputation or laceration hazards due to lack of adequate guarding on the point of operation of the Rosenthal Sheeter Model material cutter. (b) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, employees were exposed to crushing hazards due to lack of adequate guarding on moving parts (such as the cam shaft) of the Stimpson 83GW Grommetter. (c) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, employees were exposed to in-running nip points due to bypassing a light curtain sensor on the Orca Pro-Tech Laminator. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, the employer did not establish a program consisting of energy control procedures prior to employees performing servicing or maintenance on printing presses, guillotine cutters, laminators, or other automated machinery in the fabrication area. Abatement Note: Abatement certification and documentation is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) C2 Imaging, LLC: On or about August 9, 2017, and at times prior, sufficient access and working space was not provided and maintained in front of an electrical panel on the second floor next to the sewing machines. Thirty-six (36) inches of clearance was not maintained in front of the electrical panel due to a pallet of material being stored directly in front of the panel.
