29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about February 6, 2020, and at times prior, C2 Imaging LLC, as exposing and controlling employer, did not ensure its employees and employees from PrimeSource Staffing, LLC, staffing agency, were protected from struck-by and crushing injuries, in that the forklift operator was not trained in the proper operation of the forklift he was assigned to use, and the operator's competency to operate the forklift was not evaluated prior to assignment to use the Yale ERC050RGN36TE084 forklift at the site. 29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) On or about February 6, 2020, and at times prior, C2 Imaging LLC, as exposing and controlling employer, did not ensure its employees and employees from PrimeSource Staffing, LLC, staffing agency, were protected from struck-by and crushing injuries, in that unstable loads on the forklift were not secured before moving them. Large piles of scrap foam board, which are slick and have air between them because each sheet is placed one at a time, were not secured before the loads were moved. A pile fell off of the forks near workers. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle may not supply a total cord- and plug-connected load in excess of the maximum specified in Table S-4: (a) On or about February 6, 2020, and at times prior, C2 Imaging LLC, as exposing and controlling employer, did not ensure that its employees and employees of PrimeSource Staffing, LLC, were protected from fire and electrical hazards from a relocatable power tap (power strip) that was powered by the Zund cutting machine, and had the Tornado Jumbo vacuum cleaner plugged into it. The relocatable power tap had a max amperage capacity of 15 amperes. The vacuum had an operating amperage of 15 amperes. Per Table S-4 the maximum load that can be placed on a receptacle with multiple outlets and rated for 15 amperes, is 12 amperes. 29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) On or about February 6, 2020, and at times prior, C2 Imaging LLC, as exposing and controlling employer, did not ensure that its employees and employees of PrimeSource Staffing, LLC, staffing agency, were protected from shock and fire hazards from the Zund cutting machine's outlet cord that was powering a relocatable power tap (power strip). The outlet cord's insulation was pulled back from the outlet exposing the energized wires inside and placing any strain on the cord directly on the terminals of the outlet.
