Large piles of scrap foam board, which are slick and have air between them because each sheet is placed one at a time, were not secured before the loads were moved. The relocatable power tap had a max amperage capacity of 15 amperes. The vacuum had an operating amperage of 15 amperes.  Per Table S-4 the maximum load that can be placed on a receptacle with multiple outlets and rated for 15 amperes, is 12 amperes. The outlet cord's insulation was pulled back from the outlet exposing the energized wires inside and placing any strain on the cord directly on the terminals of the outlet.
