29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, they were not equipped with such guards when in use:    A.       On or about 08/10/17, at 61-40 Grand Ave, Maspeth, NY   An employee used a DeWalt grinder machine to cut metal. The guard on the grind was missing.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR1903.19.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  A.       On or about 08/10/17, at 61-40 Grand Ave, Maspeth, NY   Employees installing guard rails on the second floor were not protected from falling. The employees were exposed to the hazard of falling approximately 9 ft. to the first floor from a floor opening (5' x 6') inside of the building.  ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
