OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck by the hoist and/or the roof or its components:  a) Warehouse; On or about August 11, 2017 and at times prior, the employer exposed his employees to the hazard of being struck by a chain hoist and the roof of the building or its components. The employer failed to ensure that the structure could withstand the loads and forces imposed by the hoist, exposing employees to struck-by hazards.

OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured:  a)Warehouse; On or about August 11, 2017 and at times prior, the employer exposed his employees to the hazard of being struck by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured. PVC plastic piping was used in exposed above ground locations to transport compressed air at 165 psi.
