Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 7/28/2017, employees employed by Cesar Arango were working on the roof of a residential building located at 415 Sandra Way, Winfield, MO. Four employees were engaged in roofing work approximately 10 feet above a lower level. Exposed employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.

On 7/28/2017, workers employed by Cesar Arango were repairing a roof of a residential building located at 415 Sandra Way, Winfield, MO. The ladder the workers were using to access the roof was not extended three feet beyond the landing or edge of the roof.
