29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a. Safety inspections were not made by persons capable of identifying and correcting those hazards which could result in employee injuries. As a result, corrective action was not taken with regard to Ladder deficiencies, hazardous work zones, and deficiencies in personal protective equipment.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   a. On or about August 11, 2017 at the above referenced worksite, employees were observed working in a hazardous area without a protective helmet, as required by this OSHA standard. As a result, employees were exposed to hazards associated with falling / flying objects.    There is no abatement certification of documentation required for this item.

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement:   a. On or about August 11, 2017 at the above referenced worksite, employees engaged in residential roofing were required to use a portable ladder that was resting on an uneven surface, leaning to one side, and that was not secured from tipping while accessing the roof.  Employees were thereby exposed to a fall hazard of up to nine feet.   There is no abatement certification of documentation required for this item.

On or about August 11, 2017, employees performing residential roofing operations on sloped roof were not protected from fall hazards of up to 25 feet above grade.  Baney Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1244647, citation number 1, item number 1a and was affirmed as a final order on 09/12/2017, with respect to a workplace located at 5192 Harrison Avenue, Rockford, Illinois 61108.  Baney Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1243456, citation number 1, item number 1 and was affirmed as a final order on 08/07/2017, with respect to a workplace located at 6422-6428 Oak Crest Lane, Loves Park, Illinois 61111.  There is no abatement certification or documentation required for this item.

Baney Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1243456, citation number 1, item number 1 and was affirmed as a final order on 08/07/2017, with respect to a workplace located at 6422-6428 Oak Crest Ln., Loves Park, IL 61111.
