29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.    On or about August 4, 2017 and thereafter the employer did not adequately train the employee(s) on the purpose and function of the energy control program resulting in employee(s) being exposed to energy sources that were not locked out while clearing jams, and performing set-up and adjustments. Employee(s) did not gain an adequate understanding of work to be performed under protection of the program.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about August 4, 2017 and thereafter the company did not ensure that the procedure for slat press number 6 and the blanking press was followed to render equipment safe from or isolated to prevent unexpected restart prior to the employee(s) performing set-up (servicing) in the following order:     1. Turn off and shut down the machines.     2. Physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s).     3. Affix lockout or tagout devices to each energy isolating device.     4. Verify that isolation and de-energization of the machines or equipment have been accomplished.     5. Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or  residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.    6. Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s).    On or about August 4, 2017 and thereafter the employer did not ensure that the adjustable guarding (hairpins) on slat press number 6 was adjusted to ensure an employee(s) hand could not reach through the opening into the point of operation. Employee(s) were exposed to pinch point between dies.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
