29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training would be met:  (a)  Dynamic Roofing and Construction, Inc. at 1275 Colorado Blvd., Denver, CO 80206:  On and before August 14, 2017, the employer did not develop, implement, and maintain a written hazard communications program.   The employer did not ensure that employees using adhesives and other chemicals when roofing where made aware of the nature of the chemicals in use, the hazards associated with such use, and the safety precautions needed.   Condition exposed the employees using the chemicals to respiratory, contact and other hazards while using the chemicals.  29 CFR 1926.59: Note: The requirements applicable to construction work under this section are identical to those set forth at 1910.1200  of this chapter.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  Dynamic Roofing and Construction, Inc. at 1275 Colorado Blvd., Denver, CO 80206:  On and before August 14, 2017, the employer did not ensure that employees were adequately trained on the chemicals used in the workplace. The employer did not ensure that employees using adhesives and other chemicals when roofing where made aware of the nature of the chemicals in use, the hazards associated with such use, and the safety precautions needed.   Condition exposed the employees using the chemicals to respiratory, contact and other hazards while using the chemicals.  29 CFR 1926.59: Note: The requirements applicable to construction work under this section are identical to those set forth at 1910.1200  of this chapter.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a)  Dynamic Roofing and Construction, Inc. at 1275 Colorado Blvd., Denver, CO 80206:  On and before August 14, 2017, the employer did not provide and require the use of adequate eye or face protection when employees were exposed to the hazard of being struck-by hand tools and projectiles.  Employees were observed working in close proximity to each other using hand tools, including forks, to remove the older roofing material, without adequate eye protection in place.  This condition exposed employees to the hazard of eye injuries from being struck by the tools and projectiles.
