29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  1319 Main Street, Fitchburg, MA: On or about August 27, 2025, employees were observed working on a roof with unprotected sides and edges greater than 6 feet and up to approximately 23 feet to the next lower level and were not protected by guardrails, a safety net systems or personal fall arrest systems.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   1319 Main Street, Fitchburg, MA: On or about August 27, 2025, multiple employees were exposed to fall hazards as part of their assigned duties and the employer did not ensure that employees were trained in fall protection prior to working at heights of up to approximately 23 feet during a roofing project.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  1319 Main Street, Fitchburg, MA: On or about August 27, 2025, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for employees who were performing shingle replacement on a church roof and exposed to fall hazards.
