29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) Building 276, 2nd floor, section 230-43-04: On or about 08/09/2017, the panic bar on the exit door had tape applied over the latch bolt that restricted the door from opening with ease. Employees would have needed special knowledge by using excessive force to open the door. No abatement certification or documentation required.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of band saw(s) were not enclosed or guarded: a) Building 276, Wood Shop: On or about 08/09/2017, an Invicta vertical band saw, serial #1364 was not properly guarded leaving employees exposed to a 2.5 inch opening of the blade above the table and totally open below the table. b) Building 276, Wood Shop: On or about 08/09/2017, a Delta vertical band saw serial #87B02278 was not properly guarded leaving employees exposed to an open blade below the saw table. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a) Building 276, Wood Shop: On or about 08/09/2017, a 120/208 volts, 50 amp wall mounted breaker panel, number 276-1-L-4 was damaged beneath the bottom of the enclosure that left a 1¾ inch gap, exposing employees to a possible electrical shock or arc flash hazard. b) Building 276, Wood Shop, Bay 3G-4G: On or about 08/09/2017, the faceplate to the wall mounted 120 volts light switch used to guard the internal conductors was missing screws that caused the faceplate to separate exposing employees to a possible electrical shock hazard. c) Building 276, Wood Shop, Bay 3G-4G: On or about 08/09/2017, the faceplate to the wall mounted 120 volts duplex receptacle was damaged on the bottom that caused the faceplate to separate from the enclosure exposing employees to the internal conductors. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.304(g)(5): 29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective: a) Building 276, Wood Shop: On or about 08/09/2017, the path to ground on the 50-foot yellow extension cord used with the Hitachi, serial 101056 handheld reciprocator saw was not permanent and continuous in that the male attachment plug was missing the ground prong thereby exposed employees to a possible electrical shock hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
