29 CFR 1926.753(c)(1)(iv): The operator did not assume responsibility  for those operations under the operator's direct control. Whenever there was any doubt as to safety, the operator did not stop and refuse to handle loads until safety has been assured:  A) On or about June 20, 2017, following the initial lift of a beam that was rigged incorrectly (upside down) the operator did not take control of the job until safety was assured.  The beam was rendered and re-rigged by an employee that was not a qualified rigger and no one involved in the lift had been designated as a signal person.  Upon re-hoisting of the beam, it struck the employee and/or the beam he was walking on, causing the employee to fall approximately 14 feet.
