29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) 70 Grove Street, Watertown, MA 02472, Blending room. On or about August 28, 2017, the employer did not ensure that employees working on a walking/working surface approximately 12 feet above the a lower level were protected from falling by a means of fall protection. As employees removed product from pallets, they approached an unprotected edge. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).

29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified at 29 CFR 1910.219(m) and (o): a) 70 Grove Street, Watertown, MA 02472. Panko room. On or about August 14, 2017, the employer did not ensure that an inclined belt in the Panko room was enclosed by a guard conforming to the requirements specified at 29 CFR 1910.219(m) and (o). Employees working next to the conveyor are exposed to an unguarded in-running nip point at the base of the conveyor. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
