29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about 08/06/2017, at the above addressed worksite, employees working on the top deck of a home without fall protection were exposed to fall hazards of approximately 15-feet.

On or about 07/17/2017, at the above addressed worksite, the employer did not provide or reimburse employees for personal Protective equipment such as safety harnesses and lanyards exposing the employees to fall hazards.  On or about 08/06/2017, at the above addressed worksite, the employer did not provide safety equipment such as harnesses for employees working at heights greater than 6-feet.
