29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : Employees are exposed to occupational noise. The employer is failing to protect employees from occupational noise exposure by effective administration of their hearing conservation program. Annual audiometric testing of employees had not been performed.

The employer failed to protect employees from accumulation of flammable materials. This was demonstrated when materials were allowed to be stored within three feet of the paint booth.
