Roof structure at Cabras Marine Corporation boatyard: Training on the proper use of personal fall arrest systems was not provided to workers who were on the roof structure installing panels. Personal fall arrest systems were identified loose and not properly fitted. Employees were exposed to fall hazards of 21 feet.

Roof structure at Cabras Marine Corporation boatyard: A training program was not provided to workers using the portable stepladders to gain access to the roof structure. The ladders were used in an unsafe manner that was not according to manufacturers' recommendations. Employees were exposed to fall hazards up to 16 feet.
