29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) ASRS Machine No. 4 - On or about August 21, 2017, the employer failed to cover electrical junction boxes containing screw cap-spliced wires energized by a 240-volt motor, exposing employees to an electrical hazard. 1. Conduit boxes from the General Electric Kinamatic Motor were not covered; 2. A conduit box for the curb-side over speed sensor electrical was not covered; and 3. A conduit box for the e-stop and strobe light electrical was not covered.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) ASRS Machine No. 4 - On or about August 21, 2017, the employer failed to completely encase wiring, energized by a 240-volt motor, in conduit, exposing employees to an electrical hazard. 1. Energized wiring for the curb-side over speed sensor was not completely encased in conduit; and, 2. Energized wiring for the e-stop and strobe light electrical was not completely encased in conduit.
