29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:(a) 1) Christian Pharmacy, 4980 Phillips Drive, Forest Park, GA: On or about August 18, 2017, the employer exposed employees to a fall hazard, while employees were using an 8 foot Louisville A-frame ladder to access the upper level of the roof and the ladder did not extend 3 feet above the landing. The ladder was also not tied to the building or structure.
