Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 8/14/2017, employees from COPAL Construction, Inc. were removing roofing materials from the roofs of residential buildings located at 161-166 Harbor View Drive in Lake St. Louis, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   Three employees were observed on a 5/12 sloped roof approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.
