29 CFR  1926.20(b)(2):Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  At 606 Lynn Haven, Hazelwood, Mo. - On or about November 10, 2021, a subcontractor of Copal Construction LLC, was replacing shingles and doing work on the decking of a residential structure and the general contractor or controlling employer, did not conduct an adequate inspection of the job site, materials, and equipment.  Employees were exposed to the following hazards:  The subcontractor was not utilizing guardrail systems, a safety net system, or personal fall arrest systems.  The crew foreman for the subcontractor and three other employees were observed working on the roof, approximately 9.5 to 15.7 feet above the ground without utilizing fall protection.
