29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: Modular Storage Magazine Project, Naval Base Guam: A half-mask elastomeric respirator with particulate filters was stored hanging on an aerial lift where it was exposed to contamination and damage from sunlight, dust, moisture, and extreme temperatures. Employees were exposed to respiratory hazards.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: Modular Storage Magazine #26: The change in elevation was 20.75 inches where employees were stepping from the ground to the wooden platform to get to their gang boxes. Employees were exposed to trip and fall hazards.
