29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about 21 September 2017 and at times prior thereto, the employer did not provide a method of machine guarding in the following instances:  (a) In the back of the shop, an employee was operating a Chicago Dreis & Krump press brake without machine guarding in place, exposing the employee to the hazard of being struck by the machinery.   (b) In the back of the shop, an employee was operating a Jet Tilt Table saw that was missing the blade guard, exposing the employee to the hazard of being struck by the rotating blade.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(g) and (h) will be met:  1910.1200-(g) Safety data sheets;  1910.1200-(h) Employee information and training;  On or about 21 September 2017 and at times prior thereto, the employer did not develop or implement an effective written hazard communication program at the workplace where employees were using hazardous chemicals such as, but not limited to Oatey Heavy Duty Cement and Rubbing Alcohol, exposing the employees to the hazards of absorption, irritation, and dermal contact.
