29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts. (a) Stator Test Stations: The employer did not insure adequate guarding was provided on the Stator positioner to protect employees from exposure to the ingoing nip point created by the four trunnion rollers and stator placed on the mechanical positioner. This exposed employees to a caught-in/crushing hazard and amputation injury.

29 CFR 1910.333(a)(2): Where exposed electrically charged parts were not de-energized safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which were energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards: (a) Rotor Test Station: On or about August 14, 2017, electrical safe work practices were not properly employed to prevent exposure to energized electrical parts while conducting electrical quality tests. This exposed employees to an electrical shock hazard. (b) Stator Test Stations: Electrical safe work practices were not properly employed to prevent exposure to energized electrical parts while conducting electrical quality tests. This exposed employees to an electrical shock hazard.

This exposed employees to an electrical shock hazard at or around the rotor test station. (b) Stator Test Stations: Barricades were not effectively installed in a manner that prevented access to the stator that is being energized with 5000 volts AC for testing of electrical components. This exposed employees to an electrical shock hazard at or around the stator test station.
