29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:    a) Production     The employer's written workplace hazard assessment did not identify the person conducting the assessment and the date the assessment was performed.  In addition, the assessment did not include the cleaning of equipment, including but not limited to, Hayssen Ultima ST Bag Filler, by temporary staff.      Condition observed on or about 11/13/2017

29 CFR 1910.147(c)(4)(i): Procedures were not documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Production Area    The employer did not document and utilize hazardous energy control procedures during tool change on equipment, including but not limited to, Hayssen Ultima ST Bag Filler #18 (serial #887462) which had multiple energy sources including electrical and pneumatic.   On 8/22/17, a machine operator was assisting a mechanic during a tool change on Hayssen Bag Filler #18 when the machine started unexpectedly.  The mechanic suffered an amputation of several fingers on both hands.       Condition observed on or about 8/24/17

29 CFR 1910.147(c)(7)(iii)(A):     Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures.    (a)  Production area -Flour room    The employer did not provide training on the recognition, type and magnitude of hazardous energy sources and means of energy isolation to machine operators performing maintenance on equipment including but not limited to Hayssen Ultima ST Bag Filler.  On 8/22/17, a machine operator was assisting a mechanic during a tool change on Hayssen Ultima ST Bag Filler #18 (serial #887462) when the machine started unexpectedly.  The mechanic suffered an amputation of several fingers on both hands.    (b)  Production area - Flour room    The employer did not provide training on the recognition, type and magnitude of hazardous energy sources and means of energy isolation to temporary employees performing cleaning of the interior of equipment, including but not limited to Hayssen Ultima ST Bag Filler.    Condition observed on or about 9/20/17
