29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    Employees who perform servicing and maintenance on Centrifuge Dryers were not required to;    1. Notify affected employees of the servicing and maintenance on the dryer.  2. Turn off and shut down the machine.  3. Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source.  4.  Affix lockout devices to electrical and pneumatic energy sources.  5. Prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine have been accomplished.      On August 18, 2017 in the Dryer Room Area, Deck 5, Dryer #8,  a maintenance employee was exposed to an amputation hazard while inspecting a pulley and belt on a Heinzen Centrifuge Dryer when he moved his hand up underneath where a pulley and belt were located and about this time the dryer machine started operating.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  On August 18, 2017, in the Dryer Room,  a maintenance employee was exposed to an amputation hazard when reaching underneath an inadequately guarded pulley and belt located on Centerfuge Dryer Deck #5, Dryer #8 motor, approximately 18 inches off the ground.  Dole Fresh Vegetables, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 110715, citation number 1, item number 1 and was affirmed as a final order on December 22, 2016, with respect to a workplace located at 600 Benjamin Drive, Springfield, OH.
