29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about August 24, 2017, the employer failed to ensure that employees engaged in roofing activities including tear off and shingling were protected by flying particles. Employees were utilizing pneumatic air guns without the use of safety rated glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about August 24, 2017, the employer failed to ensure that employees engaged in roofing activities of a residential home were protected from falling to the ground below by use of fall protection devices. Employees were working at heights varying between 10 and 25 feet on a roof without the use of fall protection devices.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about August 24, 2017, during the installation of residential roofing, the employer failed to ensure that an aluminum ladder which was being used to access the roof, was extended at least 3 feet above the upper landing surface. The approximate height of the upper landing access point was 8 feet.
