On or about January 11, 2023 at the address of 5088 Williams Ct. Sheffield Lake OH 44054: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

On or about January 11, 2023 at the address of 5088 Williams Ct. Sheffield Lake OH 44054: The employer did not ensure that employees were protected from head injury hazards with the use of protective hardhat's while working on the ground below employees above engaged in roofing work activities (re-shingling/ installing underlayment material). Employees operating in this condition were exposed to struck by injuries from falling/flying objects such as tools and roofing materials.

29 CFR  1926.1053(b)(1): The ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that will not deflect:  a)	Roofing Solutions Joe LLC. On or about January 11, 2023 at the address of 5088 Williams Ct. Sheffield Lake OH 44054: The employer did not ensure the aluminum ladder had its side rails extended at least three feet above the landing surface. Four employees performing roofing work activities were exposed to fall hazards while using ladder to access/exit roof on a two-story residential structure with a height of 11ft - 7inches from ground below.

On or about January 11, 2023 at the address of 5088 Williams Ct. Sheffield Lake OH 44054: The employer did not provide a training program for each employee potentially exposed to fall hazards while using an aluminum extension ladder to access a two-story residential roof structure. Employee were not trained to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)	Roofing Solutions Joe LLC. On or about January 11, 2023 at the address of 5088 Williams Ct. Sheffield Lake OH 44054: The employer did not ensure employees were protected from falls while performing roofing work activities (re-shingling/installing underlayment material) atop a two-story residential structure with an 8 -12 roof pitch. Employees operating in this conditions were exposed to falls of  30ft to the ground below.  Roofing Solutions Joe LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1616733, citation number 1, item number 1 and was affirmed as a final order on December 1, 2022, with respect to a workplace located at 9536 Ridge Rd. North Royalton OH 44133.
