29 CFR 1926.351(b)(4): Welding cable(s) in use were in need of repair:    Building 2 Mock Search House - EODMU-5 Complex: The insulating jacket on the cable for the Deluxe Inverter DC ARC Welder was pulled back from the terminal screw exposing energized copper wiring. The insulating jacket was also split, exposing internal components. Employees were exposed to electrical shock hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Building 2 Mock Search House - EODMU-5 Complex, upper platform: Employees were performing welding and sawing operations in areas where fall protection was not adequate to protect them from falling to the lower level. Employees were exposed to fall hazards up to 144 inches.   Leading Tech Construction Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 902978, citation number 1, item number 1 and was affirmed as a final order on July 19, 2013, with respect to a workplace located at Summary/Nails Ri-By, located between Ban Thai and Fuji Ichiban Restaurant, Tamuning, Guam 96913.
