29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:      a)  On August 25, 2017, employer did not provide training to each employee in order to recognize hazards associated with working at an elevation and to understand procedures to control or minimize those hazards.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.552(a)(1):  The employer shall comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:      a)  On August 25, 2017, employees were observed using the material hoist as a ladder to access and egress the roof.     No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On August 25, 2017, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 18 feet above the ground.    Midwest Renovation Services, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 763722, citation number 1, item number 2 and was affirmed as a final order on January 2, 2013, with respect to a workplace located at 729 S. Kenilworth Ave., Oak Park, IL 60304.    No abatement certification or documentation is required for this item.
