Midwest Renovation Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1258432, citation 2, item 1, and was affirmed final order on October 4, 2017, with respect to a workplace located at 517 Cobblestone Ln., Buffalo Grove, IL 60089.      Midwest Renovation Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 763722, citation 1, item 2, and was affirmed final order on January 2, 2013, with respect to a workplace located at 729 Kenilworth Ave., Oak Park, IL 60304.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
