29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On May 17, 2018, employees performing roofing work were exposed to fall hazards of approximately 21 feet.    Midwest Renovation Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1258432, citation 2, item 1, and was affirmed final order on October 4, 2017, with respect to a workplace located at 517 Cobblestone Ln., Buffalo Grove, IL 60089.      Midwest Renovation Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 763722, citation 1, item 2, and was affirmed final order on January 2, 2013, with respect to a workplace located at 729 Kenilworth Ave., Oak Park, IL 60304.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  On or about May 17, 2016, employees were exposed to fall hazards while performing roofing work and the employer did not provide a training program to recognize fall hazards and the procedures to be followed in order to minimize those hazards.      Midwest Renovation Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1) which was contained in OSHA inspection number 1258432, citation 1, item 1, and was affirmed final order on October 4, 2017, with respect to a workplace located at 517 Cobblestone Ln., Buffalo Grove, IL 60089.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
