29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.      Building 101A, Naval Base Guam: An employee was not using eye protection when working with Portland Cement, was exposed to caustic burns to the eyes.

29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical.   Building 101A, Naval Base Guam: An outdated copy the material safety data sheet for Portland Cement was maintained at the jobsite. The employer did not have the updated safety data sheet available for employees working with the cement mix. Employees were exposed to chemical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   Building 101A, Naval Base Guam: Electrical cords that provided power to the lights had their jackets pulled back from the plug end, exposing internal wiring. Strain relief was not provided. Employees were exposed to electrical shock hazards.
