The Tuber area rubber pellet hopper had an elevated platform vertical ladder without swing gate protection.

The Jedelshon, Blue Lever and Green Lever slitter machines had 18 inch diameter metal blades that were inadequately guarded.    b. The saturation room has two rolling mills that apply molten tar onto fiberglass and cotton bales. The rolling mills had molten asphalt tanks, rotating rollers and rotating projected shaft ends that were inadequately guarded.
