29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm): (a) On or about August 28, 2017 at 200-11 46th Avenue, Flushing, NY 11220 Employees installing wooden 2x4's from tubular welded frame scaffold that was not fully planked at working level. The working level was approximately 20 feet from the lower level. WRITTEN ABATEMENT CERTIFICATION IS REQUIRED.
