29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    At a residential construction site located at 933 Amber Ct, Blue Springs, MO: Employees were exposed to fall hazards. The employer failed to protected employees from fall related hazards in that employee were not trained on recognizing fall hazards and avoidance of the unsafe conditions. This was demonstrated when employees were observed working at a height 10.7 feet with the use of fall protection equipment that was improperly donned and affixed.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    At a residential worksite located at 933 Amber Ct, Blue Springs, MO: Employees were exposed to fall hazards. The employer failed to provided safe access to an upper landing for employee. Employees were observed using a portable ladders to gain access to a roof, and that ladder was only placed 2 feet above the upper landing that was being accessed.

The employer failed to protected employees from fall related hazards in that employees were observed working at a height of 10.7 feet without the use of any protective systems.    United Roofing Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 1926.501(b)(13), which was contained in OSHA inspection number 921241, citation number 1, item number 2 and was affirmed as a final order on 09/24/2013, with respect to a workplace located at 3104 Monterrey Dr., Blue Springs, MO.    United Roofing Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 901633, citation number 1, item number 2 and was affirmed as a final order on 07/03/2018, with respect to a workplace located at 9200 North Oak Trafficway, Kansas City, MO.
