29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: At a site located at 2200 Northeast Town Centre Boulevard Lees Summit, MO: Employees were exposed to struck-by hazards. Employees were using pneumatic nail guns when putting down roofing shingles without the use of any form of safety glasses. Employees were exposed to struck-by hazards when eye protection was not provided or worn at the time of the inspection

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the residential construction site located 2200 Northeast Town Centre Boulevard Lees Summit, MO: Employees engaged in residential roofing activities without the use of any form of fall protection. Approximately 25 employees were exposed to fall hazard of approximately 17 feet with a 4:12 pitch. Numerous employees were seen wearing a personal fall harness, but only one employee was attached to a rope incorrectly wearing his PFAS and only 3 anchor points on one building with eight employees. United Roofing Services was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in osha inspection number 1394765, citation number 1, item number 1 and was affirmed as a final order on July 25, 2019 with respect to a workplace located at 225 SW Carson St Blue Springs, MO 64013. United Roofing Services was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in osha inspection number 1259251, citation number 1, item number 3 and was affirmed as a final order on March 25, 2019 with respect to a workplace located at 933 Amber Court, Blue Springs, MO 64014
