29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) Jobsite - On or about August 29, 2017, a personnel platform attached to a JCB forklift (Model 507-42) that was not designed to support personnel platforms was used to elevate employees to the roof of a building, thus exposing employees to fall hazards of up to about 20 feet onto earthen ground.

29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: a) Roof - On or about August 29, 2017 - An employee stood on the guardrails of the basket of a scissor lift to access the roof of the building, which was more than 14 inches above the scissor lift basket, thus exposing the employee to falls of up to about 20 feet onto earthen ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Roof - On or about August 29, 2017, the employer did not ensure that employees were protected by safety net systems or personal fall arrest systems while installing gutters at the edge of the roof of the building with sides that were unprotected by guardrail systems, thus exposing employees to fall hazards up to about 20 feet onto earthen ground. Superior Metals Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1185311, citation number 1, item number 1 and was affirmed as a final order on January 9, 2017, with respect to a workplace located at 4054 I-17 South, Ashburn, GA 31714.
