29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.   a) 62 Kingwood Dr., Little Falls, NJ:  Employees were exposed to fall hazard in excess of 10 feet while working on the first level roof of residential construction without use of guard rails, safety nets, or personal fall arrest systems, on or about 08/30/17.  b) 62 Kingwood Dr., Little Falls, NJ:  Employee was exposed to fall hazard in excess of 18 feet while working on the second level roof of residential construction without use of guard rails, safety nets, or personal fall arrest systems, on or about 08/30/17.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 62 Kingwood Dr., Little Falls, NJ: Employees were exposed to fall hazard of approximately 10 feet while using an extension ladder to access a roof. The extension ladder that was used did not extend at least 3 feet above the landing roof surface, on or about 08/30/17.
