29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  Carework Construction LLC, as successor to Carework Construction LLC, and Jose Gabriel Sucuzhany does not ensure that employees use adequate fall protection when working at elevated heights.. This violation was most recently observed at 451 Wyckoff Avenue, Wyckoff, NJ, at instance (a): Employees engaged in carpentry activities were exposed to a fall hazard of approximately 14 feet to an open trench as a means of fall protection had not been utilized, on or about 7/22/2020. To abate this violation the employer must ensure that its employees performing roofing activities re protected from fall hazards and that no employee is allowed to be exposed to fall hazards without utilizing adequate fall protection.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Carework Construction LLC, as successor to Carework Construction LLC, and Jose Gabriel Sucuzhany  does not ensure that employees use appropriate head protection where employees were exposed to overhead hazards. This violation was most recently observed at 451 Wyckoff Avenue, Wyckoff, NJ, at instance (a): Employees engaged in carpentry activities were exposed to an overhead impact hazard overhead work was taking place, as the employer failed to ensure the use of head protection was utilized. Condition noted on or about 7/22/2020. To abate this violation the employer must ensure that its employees performing carpentry activities with work overhead are protected from head injury and that no employee is allowed to be exposed to head injuries without wearing proper head protection.  Carework Construction LLCwas previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.100(a):  which was contained in OSHA Inspection Number 1455338, Citation # 1, Item # 1, issued on 03/03/2020 with respect to a work place located at 244 Everett Place, Wyckoff NJ 07481. This citation became a final order on 05/22/20.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Carework Construction LLC, as successor to Carework Construction LLC, and Jose Gabriel Sucuzhany does not ensure that employees use appropriate eye protection when exposed to eye injury hazards, including but not limited to when using tools such as a circular electric saw, a pneumatic nail gun and a hammer. This violation was most recently observed at 451 Wyckoff Avenue, Wyckoff, NJ, at instance (a): Employees engaged  in carpentry work using  pneumatic guns were exposed to struck-by hazards with the potential to cause eye injuries or loss.  The employer did not ensure the use of appropriate eye protection for these employees, on or about 7/22/2020. To abate this violation the employer must ensure that its employees performing roofing activities are protected from eye injury and that no employee is allowed to be exposed to eye injuries without wearing proper eye protection.
