29 CFR 1926.502(b)(13): A guardrail systems was not used around holes which were used as points of access (such as ladderways), they were not provided with a gate, or were not offset so that a person cannot walk directly into the hole: a) South building: On July 13, 2107 and at times prior thereto a floor hole without a guardrail system in place, was used by employees for access to the upper level. This exposed the employees to a ten foot fall hazard. Note) Corrected during the inspection.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: a) South building, 4th floor: On July 13, 2017 employees used an extension ladder to get to an upper level. The extension ladder did not extend 3 rungs above the landing. Employees were exposed to a 10 foot fall while getting on and off of the ladder. b) South building, roof deck: On July 13, 2017 employees used a ladder for access to the roof deck work area. The ladder did not extend 3 rungs above the roof deck. This exposed employees to a 10 foot fall while getting on and off of the ladder. Note) Corrected during the inspection.
