29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Water house area - On or about August 28, 2017, authorized employees were required to remove a jam of the plastic tray(s) and also clean the Collussi No. 1 and No. 2 machines. The written energy control program did not include step-by-step lockout procedures utilizing the new key system (modified trap key system). Employees were thereby exposed to the hazards of inadvertent start-up of the equipment

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    1. Water house area - On or about August 28, 2017, authorized employees (janitors) were required to remove debris from the out board trays and other areas of the Collussi No. 1 and No. 2 machines. Lock(s) were not provided for isolating, securing or blocking of machines or equipment from energy sources. Employee(s) were thereby exposed to the hazards associated with inadvertent start-up of the equipment.    2. Water house area - On or about August 28, 2017, an authorized employee (a Lead) was required to remove a jam of the plastic tray(s) from the Collussi No. 1 and No. 2 machines. Lock(s) were not provided for isolating, securing or blocking of machines or equipment from energy sources. Employee(s) were thereby exposed to the hazards associated with the inadvertent start-up of the equipment.

Water house area - On or about August 28, 2017, an authorized employee (a Lead) was required to remove a jam of the plastic tray(s) from the Collussi No. 1 and No. 2 machines. The authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and methods and means necessary for energy isolation and control.    2.) 29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    Water house area - On or about August 28, 2017, supervisor(s), lead(s) and maintenance were required to work together to remove a jam (25 trays or more) inside the machine. Each authorized employee (a supervisor and a lead) did not affix a personal lockout or tagout device to the group lockout device (the new key system) before working on the machine or equipment. Employees were thereby exposed to the hazards of inadvertent start-up of the equipment.
