29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:  Site- Employees were not protected from a fall and scaffold collapse hazard where a ladder supporting a ladder jack scaffold was not able to support the intended load without settling. On or about August 29, 2017 a Louisville AE2824 extension ladder was used to support part of a ladder jack scaffold. The bottom of the ladder settled when loaded causing the bottom of the ladder to sink into the ground and the top of the ladder to fall off of its building support. Two employees fell off of the ladder jack scaffold as a result of the unintended scaffold movement.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Site- Employees exposed to fall hazards while working from a roof and ladder jack scaffold where not provided with fall protection training from the employer. On or about August 29, 2017 an employee who had not received any fall protection training from the employer did not attach themselves to a life line when accessing a ladder jack scaffold and fell from the scaffold.    Pride Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.132(f)(1), which was contained in OSHA inspection number 1041939, citation number 1, item number 1d and was affirmed as a final order on April 20, 2015, with respect to a workplace located at 65 Portsmouth Ave, Stratham, NH 03885.
