29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Norfolk Naval Shipyard, Scotts Center Annex, Building 1717 Deldonna St., Portsmouth, VA 23709: On or about August 31, 2017, and times prior thereto, the employer did not implement use of a written energy control procedural element, where authorized employees failed to apply a lockout device to equipment, prior to disassembly and cleaning of equipment including, a) a Biro band saw, S/N 49495, b) a Biro band saw, S/N 49496, c) a Hollymatic 120 Grinder/Bulker, S/N 7005, d) a Hollymatic 120 Grinder/Bulker, S/N 7030, where employees used the energy control procedures on a frequent/daily basis, exposing employees and contractor employees to caught-in and/or electrical hazards. e) the employer's energy control procedures were not written in step-by-step procedures specific to the machine, such as J.V. Manufacturing Cram-O-Lot balers, where contractors performed servicing and maintenance, exposing contractor employees to potential caught-in hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
