29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a) In the Inspection Area, prior to changing abrasive belts on the Burr King Belt Grinder (Model #760), employees did not utilize the established energy control procedure.  These machines were cord and plug, however employees were shutting off the machines using an E-Stop and were not unplugging them prior to changing the abrasive belts.  This condition exposed workers to the hazard of unexpected machine start up and amputation injury.
