29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Melt Deck Module #4: On or about July 3, 2025, employees working on the melt deck were exposed to a crushing hazard, when the raised furnace melt deck floor rotates and lowers back into the down position after completing a pour. The crushing hazard exposure occurs at the pinch point created between the furnace melt deck floor and stationary concrete melt deck floor.
