29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) American Modern Exteriors, LLC failed to provide documentation demonstrating the abatement method and completion of the following items as cited:      a) Citation 1, Item 1a: Establish and maintain a program to address job site safety and health and provide a competent person to address recognized hazards for workers.     b) Citation 1, Item 1b: Ensure that fall protection is provided and utilized during construction work at heights greater than 6 feet.      c) Citation 1, Item 2: Ensure ladders used on construction site are in good condition and properly setup having side rails extend three feet above the landing surface level.

29 CFR 1926.20(b)(2):   It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On September 1, 2017 at 104 West Main Street in St. Nazianz, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized the fall from elevation hazard associated with employees working at open roof edges without utilizing fall protection equipment.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a)  On September 1, 2017 at 104 West Main Street in St. Nazianz Wisconsin, an employee was working on a residential roof surface 10 ft. above surface level without utilizing fall protection equipment.  b)  On September 1, 2017 at 104 West Main Street in St. Nazianz Wisconsin, an employee was working on a garage roof at the ridge and gable ends 10 ft. above surface level without utilizing fall protection equipment.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  On September 1, 2017 at 104 West Main Street in St. Nazianz Wisconsin, an employee was using a multi-use ladder to access/egress a roof surface without the side rails at least 3 ft. above the roof surface.
