29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):      a)  On August 31, 2017, employees were exposed to the hazard of falling approximately 6 feet to the ground, while performing masonry work from tubular welded frame scaffold platforms that were not fully planked.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On August 31, 2017, employees engaged in masonry activity were not protected from fall hazards while working at an elevation of approximately 27 feet above the ground.      b)  On August 31, 2017, employees were exposed to the hazard of falling approximately 27 feet to the ground while working in the hoist area.       No abatement certification or documentation is required for this item.

29 CFR 1926.602(d): Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178:        29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):       a) On or about August 31, 2017, the employer did not train and evaluate employees operating the Ingersoll Rand VR-843C all-terrain forklift.        D.C.A. Quality Construction Co. Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 749843, citation number 1, item number 1 and was affirmed as a final order on December 27, 2012, with respect to a workplace located at 9220 Crawford, Skokie, IL 60076.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 749843, citation number 1, item number 2b and was affirmed as a final order on December 27, 2012, with respect to a workplace located at 9220 Crawford, Skokie, IL 60076.
