On or about July 31, 2017, the employer exposed employees to struck-by and pinch point hazards by not developing and implementing specific procedures for authorized employees who applied lockout devices to the Stamco machine (Machine #4455, SN: 407 84 17A) when making changes and/or adjustments to the rolls.
