29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       a) On or about September 5, 2017, employees were exposed to the hazard of falling approximately 11 feet where a portable extension ladder did not extend above the landing surface a roof deck.        In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.300(b)(4)(ii): The point of operation of machine(s) whose operation exposed an employee to injury was not guarded:         a) On or about September 5, 2017, the cutting blade on a Ridgid electric circular saw was not guarded when used to cut materials at the site.     b) On or about September 5, 2017, the cutting blade on a Bosch electric Miter saw was not guarded when used to cut materials at the site.    E & G was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.300 (b)(1) which was contained in OSHA inspection number 1140706, citation 1, item 1, and was affirmed as a final order on August 3, 2016, with respect to a worksite located at 622 Cedar Street, Park Ridge, Illinois.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

E & G installation Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1140706, citation 1, item 3b, and was affirmed as a final order on August 3, 2016, with respect to a worksite located at 622 Cedar Street, Park Ridge, Illinois.
