29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) United Parcel Service at 400 W. 69th Street, Loveland, CO 80538: On or before August 30, the employer did not ensure that procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. The guards were removed on conveyors, such as, but not limited to, the PS-1 conveyor, to remove envelopes caught in the conveyor, caught inside the conveyor guarding, or caught on the conveyor rollers. This condition exposed employees to amputation hazards associated with ingoing nip points and rotating parts.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed: (a) United Parcel Service at 400 W. 69th Street, Loveland, CO 80538: On or before August 30, the employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed. The employer's lockout tag out procedure for conveyors provided that lockout is to be used when repairing, maintaining, cleaning, or any other task in which the release of hazardous energy is possible. This procedure provided that lockout is to be used when a package must be retrieved that is caught in a conveyor or inside the conveyor guarding. The employer did not conduct a periodic inspection of the energy control procedure to ensure that it was properly understood and used. This condition exposed employees to amputation hazards.
